Smile lines, also known as nasolabial folds, are the wrinkles extending from your nose to the corners of the mouth. This is one of the main effects of aging and they’re due to appear on most, if not all, faces. As we strive to rejuvenate our youth, smile lines are looked at as a major pain point, and if reversed, would make you look years younger. One of the latest advancements in this field is the use of polydioxanone (PDO) threads, a minimally invasive procedure gaining popularity for its effectiveness in treating smile lines through the use of lifting and collagen production.
PDO threads are a type of medical-grade thread used in thread lift procedures. They are made from polydioxanone, a biodegradable material that has been used in surgical sutures for decades. When inserted under the skin, they provide a dual benefit: immediate lift and collagen stimulation.

When it comes to the type of PDO threads for nasolabial folds, there are three main types that are used:
Bi-directional barbed thread: These threads give the primary lift, using the barbs to latch onto the skin and reposition to smooth out smile lines. The ideal lifting thread will have press molded barbs, in which barbs are molded onto the thread, as opposed to cut threads which have barbs cut from the thread. They are usually long enough to insert from the hairline and down towards the nasolabial fold to pull the sagging tissue around your cheeks to relieve the smile line.
Multi-directional barbed thread: This type is used in conjunction with the bi-directional variant. The barbs constantly alternate directions along the whole length of the thread, and are used to fix and secure the bi-directional threads (like double-sided sticky tape) for additional lifting support. This also limits the excessive movement of tissue to extend the longevity of the lift.
Smooth threads: These threads are completely smooth with no barbs, and are typically shorter than its lifting counterparts. They are generally used in bunches and the way they’re placed all depends on what you’re trying to achieve (parallel, perpendicular, cross, circular, etc.). Barbed PDO threads provide lifting and collagen stimulation, whereas smooth threads offer collagen stimulation and additional volume, hence why it’s also called “collagen threads”.
The tools used in your aesthetic procedures are just as important as the skill level of your provider. Make sure your provider is using the best brand of PDO threads. Some key points to look out for are:
- Instant Results: The lifting effect is visible immediately after the procedure.
- Collagen Production: PDO threads stimulate collagen production, improving skin elasticity and texture over time, specifically the smooth “collagen threads.”
- Minimal Downtime: Unlike surgical facelifts, PDO thread lifts require minimal recovery time.
- Natural-Looking Results: The procedure can provide a natural-looking lift, avoiding the “overdone” appearance.
PDO threads are also great in combination with other procedures. While thread lifts can help with underlying sagging tissue, injectables like fillers and neurotoxins can address loss of volume and wrinkles to address aging related issues with minimally invasive treatments. Then skincare treatments like skin boosters, microneedling, and facials can be used to hydrate and nourish the surface of the skin for comprehensive facial rejuvenation. Ideal candidates for PDO thread lifts are those experiencing mild to moderate sagging skin, particularly around the nasolabial folds. It can be most effective for individuals in their 30s to 50s, who are not quite ready to undergo a surgical facelift and are looking for a less invasive alternative.
While PDO threads are generally safe, it’s crucial to have them performed by an experienced professional. Possible side effects include swelling, minor bruising, discomfort, dimpling, etc. It’s also essential to have realistic expectations and understand that the results are not permanent, typically lasting around 10 months.
